By registering, I acknowledge that wrestling is a contact sport that involves physical risk. I grant permission for my child to participate and release the tournament organizers, Matside Wrestling Co, and all affiliated parties from liability for injuries sustained during participation.

I certify that my child is in good health and physically able to participate. I authorize emergency medical treatment if necessary.
